This article provides answers to some frequently asked questions regarding benefits checks and coverage verification as it relates to funding for communication devices.

Can I check my insurance benefits on my own?

Yes!  Families and clinicians are encouraged to contact the insurance company directly to verify benefits. This is often the fastest way to obtain accurate information.

How do I check my benefits coverage for a communication device?

Call the Member Services phone number on the back of your insurance card and ask the representative the questions listed below.

• Is Forbes Rehab Services, Inc. in network with my insurance?
  • They may ask for our National Provider Identifier (NPI): 1346326220
• Is a speech-generating device a covered benefit in my plan?
  • They may ask for the high-tech speech generating device (SGD) billing code: HCPCS Code E2510

Will I have out-of-pocket costs?

To help you estimate your potential out-of-pocket cost, we recommend confirming the following with your insurance company:

• Your deductible
• Your coinsurance or copayment

These amounts may differ depending on if Forbes Rehab Services is in-network or out-of-network with your insurance provider.  

Unfortunately, it can be difficult to estimate exactly what your out-of-pocket costs will be, but Forbes AAC is committed to helping to ensure that high out-of-pocket costs are never a reason that someone is denied access to a voice. What if I have Medicaid?

If you have Medicaid, the speech-generating device is a covered benefit.

Please note:  Medicaid requires providers to be enrolled and credentialed in the state where the patient resides. Forbes Rehab Services must be an approved Medicaid provider in your state in order to provide services and bill Medicaid. Coverage and availability may vary by state.
