How do I make a Loan Library request? – Forbes AAC Online Help Center

This article covers how to make requests for Clinician Loans, End User Loans, and Evaluation Kit Loans! Are you curious about the different loan types? Refer to this article for more information: What types of loans are available from Forbes AAC?

Please note that all loans must be initiated by an SLP.

Option 1: Clinician Loan

Ideal for device evaluations in your clinic or preparing for client trials (the device must stay with the SLP and cannot be sent home with patients)

Visit the Forbes AAC Loan Library Webstore
Log in to the Webstore (or create an account if you haven't already) by clicking "Account" in the upper right corner of the screen. You will see a dropdown menu that allows you to...
1. Log in if you are a returning customer, or
2. Create an account if you have not already done so.
Please Note: This is a separate login from your Funding Portal account! When you create a Loan Library Webstore account for the first time, you will receive an account verification email to the email address you used to create your Webstore account. Add a device and keyguard/mount (if applicable) to your cart.
When you're ready to check out, click "View cart" in the upper right corner of the screen.
From your cart...
1. You can adjust the loan duration if needed (maximum of 4 weeks).
  1. Please note: If you are hoping to have a device by a specific date, you can select a date at this stage; however, please also reach out to your local Assistive Technology Specialist. While specific delivery dates cannot be guaranteed, they will do their best to assist with coordinating prompt delivery.
2. Fill out the following boxes:
  1. Recommending SLP: Your name
  2. Licensure Number: Your SLP state license number.
  3. Clinician Loan or Trial Device: Specify that this is a "Clinician Loan."
  4. Forbes AAC ATS: Select your local Assistive Technology Specialist from the dropdown.
  5. Keyguard Layout: If you are ordering a keyguard, be sure to select your preferred configuration.
    1. If you are ordering a keyguard ONLY for an existing device you already have on loan, please put the serial number of that device in the "Additional order notes" section.
  6. Mounting Plate: Specify whether or not you need a mounting plate and which type (available for all devices except the ProSlate 6 MAX)
  7. Additional Order Notes: Any additional order notes that may be helpful to our team.
  8. Do not fill out "Benefits Check Approval Code," "End User Name," or "End User Email Address." These boxes are only for "End User Loans" (not "Clinician Loans").
When you've added all of the required information, agree to the "Terms and Conditions," and then click "Checkout" in the lower right corner of the screen.

You will receive an email when the request has been approved and when the device is ready to ship, but you can also track your order progress at any time by logging into the Loan Library Webstore!

Option 2: End User Loan

Ideal if the patient/family would like to trial the device outside of the therapy session (this device can go home with an individual patient/family)

Begin by submitting a Benefits Check via this link Benefits Check or by selecting the "Submit New Loan Device" button on the Funding Portal:

You will need to create a Funding Portal account to complete this step if you don't have one already!

1/6/2025 Update: Users who created Funding Portal accounts before December 31, 2025 must re-create their login credentials to continue accessing the Funding Portal. Please use the same email address you previously used, as all account data and funding packets are tied to that email and will remain available.
The family will automatically receive an email request to sign a Release of Information (ROI) via DocuSign.
Our team will complete a benefits check to ensure that a high-tech device is a covered benefit under the patient's insurance plan.
If a high-tech AAC device is a covered benefit, you (the SLP) will receive an email with an approval code that you can use to request the device.
Once you have the code, visit the Forbes AAC Loan Library Webstore (this will also be linked from the email).

  10. End user email address: Enter the patient (or parent/guardian/authorized caregiver's) email address.
When you've added all of the required information, agree to the "Terms and Conditions," and then click "Checkout" in the lower right corner of the screen.

Option 3: Evaluation Kit

Ideal if you already have iPads with software, and would just like to place them in a durable case with a speaker and handle!
